Creating final Merge Patch

The guide telle us to make an aditional merge patch to cover those areas the bashed patch didn't. Should I include every plugin in my load order, incluiding the bashed patch?

I've always created the merged patch first and place Merged Patch.esp toward the bottom of the load order. Once the merged patch is created, I create a bashed patch (and you can include the merged patch in the bashed patch) and place Bashed Patch, 0.esp below Merged Patch.esp.
